Carnival now has it in with our Cruise Documents/boarding pass. We can now print them out and have them filled out prior to boarding. This has been added since I printed out boarding pass/luggage tags. Just a heads up.

I printed all my docs just now and it was there I will make copies if anyone needs one I just looked at mine and there is no name on it. I don't think I will fill it out till I get to NO
