Hey Ashley, I just read your post. I didn't realize you need that if you were over 18. From looking at other posts on this topic and reading NCL's policy, it looks like you need just a pretty general letter saying that you are allowed to travel. Here are two places you may want to look to get a good model to use for your letter.

You may want to add the statement, "I also authorize medical treatment for my daughter in case of an emergency." Then have your parents sign it in front of a notary and you should be good to go. It seems like it's more of a formality than anything. I hope this helps.
